Jack Ray Tate

Mr. Jack Ray Tate, 59, of 3236 East Pine Street, Mount Airy, passed away Saturday night, March 17, 2012, at Forsyth Medical Center in Winston-Salem. Mr. Tate was born November 17, 1952, in Surry County, the son of the late Ray and Hazel Davis Tate. He was employed at Walmart. Survivors include his wife, Barbara Jean Hayden Tate of the home; two daughters, Peggy Velazquez and Mark of Irving, TX, and Patty Tate and Stephen Moose of Mount Airy; a son, Barry Tate and Wendy of Lancaster, PA; a grandson, Corbin Moose of Mount Airy; his mother-in-law, Louise Hayden of Mount Airy; a sister-in-law and her husband, Dianne and Johnny Willey of Lowgap; and two brothers-in-law and their wives, Gary and Joanie Hayden of Mount Airy and Earl and Crystal Hayden of Westfield. The funeral service will be held Wednesday, March 21, 2012, at 2:00 PM at Moody Funeral Home Chapel in Mount Airy, with the Rev. Mark A. Reece, Jr. officiating. The family will receive friends one hour prior to the service at the funeral home, and other times at the home Patty Tate, 1117 Linville Road, Mount Airy.
